of the following is a possible cause:
a: deactivated by a console command. 
b: disk drive failure.
c: driver unload.
d: file system failure.
e: wrong medium.
f: end-of-medium.
g: end-of-medium MINOR.
h: unload command.
i: dismount command.
j: down command.
