monitoring thread in the CallManager or from a heartbeat
monitoring process.

  heartBeatStopped:          The CallManager stops generating
                             a heartbeat

                             of the router thread
  timerThreadDied:           The CallManager detects the death
                             of the timer thread

                             of one of its critical threads
